Work permit applications can be printed off from the link below. Please complete student portion of the application, have a parent/guardian sign and then have your employer take care of their part of the application prior to getting school approval.
- Work Permit Summer-Work-Permit-Application-1-1.pdf
- During summer hours of 8:00 am-12:00 pm, you may drop off at main office or email to tbarth@chicousd.net
- Return completed form to CHS main office in person to receive actual signed permit
