ASB Forms have been updated! If you have any questions, please email or stop by by to see Robyn Salyer in the Panther Post. Any forms for Executive Council need to be turned in by 8:30 a.m. on Tuesday to be approved that week. 
 
You must download  and open each form to use from the website.  Please note if using the PDF version you will need to download, open the file and click on filll and sign to type into the form. You  must also complete each section of the form as it does NOT caculate for you like it does in the Excel version. Microsoft Office and Adobe Fill in forms can be filled in and saved to your computer.  It is preferred that you type and print to turn in.   Forms completed on the computer are much easier to read and the preferred way of turning in forms.  Please note that some forms are REQUIRED to be typed as they are to be submitted to CUSD for approval.  If you choose to fill in a form by hand, you MUST use the PDF form verses the Excel version as those versions will print with zeros as they contain formulas.  Turn in completed form with the appropriate signatures to the Panther Post.
 
 
ASB P.O. Requisition Form
 
It is preferred this form is typed, but if you hand write you MUST use the pdf format, as the Excel version contains formulas.  Complete this P.O. requisition in order to get a P.O.  Make sure you have both a student and Advisor signature.  Turn in to the Panther Post by 8:30 AM on Tuesday for Executive Council that week.
 
ASB Deposit Form
 
It is preferred this form is typed, but if you hand write you MUST use the pdf format, as the Excel version contains formulas. If you have more than 20 checks in your deposit, please also complete the below Additional Checks Deposit Form. Please turn in all ASB money using this deposit form.  Make sure to have back up for your deposit.  Please have a student sign and counted by. Turn in deposits before 2 p.m. for the same day.
 
ASB Transfer Form
 
An ASB transfer form is used to make transfers from one ASB account to another ASB account within ASB.
 
ASB Fundraising Request Form
 
In order to have a fundraiser, an ASB fundraising request must be approved.  Please allow enough time for this form to also be approved by CUSD before your event. This is a minimum of 2 weeks. Turn in to the Panther Post by 8:30 AM on Tuesday for Executive Council that week.

ASB Change Request
 
If you need a change box for an event, please turn in the change request at least 3 days prior to needing the cash. 
 
ASB Independent Contractor Agreement
 
The Chico High site code is:  010.  This form is to pay an outside consultant for work/labor to be performed.  Please plan ahead.  All Independent Contractor agreements must be approved by CUSD prior to any work being completed. Please allow a minimum of 2 weeks for approval. Turn in to the Panther Post by 8:30 AM on Tuesday for Executive Council that week.

Donation to School
 
This form is used for donations to be accepted and acknowledged.
 
Chico High Clubs
 
If you would like to start a new club, please complete both the New Club form and Club Constitution.  Please bring the form to be reviewed to the Panther Post before obtaining necessary student signatures to be reviewed. This form must be typed as it is kept on file and is sent to CUSD for approval. Turn in to the Panther Post by 8:30 AM on Tuesday for Executive Council that week.
 
Chico High Marquee Requests
