Enrollment for Chico High School

Welcome to Chico High School.  Please select the link to fill out the registration form to enroll your student.  Our registrar will retrieve the records and our counseling staff will collect appropriate records and call you for an appointment with a counselor. When you meet, you will be able to ask questions, enroll in classes and be given a tour if need be.  For those with no internet access computer access is available in the counseling office of our administration building.
 
The following information is required for enrollment:
  1. Two (2) proofs of address in parent/guardian's name, for example:
    • Current Utililty Bill
    • Rental/Lease Agreement
    • Driver's License
    • Notarized Statement from owner/renter indicating the names of people living with the owner/rental
  2. Transcript and Withdrawal Grades
  3. Immunization Record w/proof of T-dap vaccine
  4. Proof of date of birth (birth certificate, birth record, passport, baptismal certificate, affidavit of the parent, guardian or custodian of the minor or other legal proof)

Students with an IEP

Students with an IEP (in addition to the requirements above), should reach out to our:  School Psychologists
The Psychologist will need:  student name, student birth date, parent names, call back number, and the name of the former school attended.

Form 10 & Form 11

Intra-District Transfers (Form 10s) are utilized by parents who currently reside within Chico Unified School District and wish to have their child attend a school of their choice rather than their neighborhood school.
 
Inter-District Transfers (Form 11s) are used for students residing outside of Chico Unified School District, but who meet residency requirement that may allow them to enroll in a Chico Unified School. The Intra-District Transfer form must begin with the school district you currently reside.
